Use this form to notify administration of any student-involved incidents.
Please ensure you are only reporting information that is true and accurate.

An administrator or counselor will follow up with you as soon as possible.

⚠️ Important: If you are reporting a fight, physical altercation, weapon, illegal substance, or threat, please alert an adult immediately.
